Data Use Consent (Required)
WOJRC is a non-profit agency that is funded by grants and donations. Your information may be shared through our database systems so that we can engage in activities such as grant applications and statistical reporting to major benefactors. This type of reporting would generally be aggregated and statistical in nature. Any such reporting would not include information that could be associated with any specific WOJRC client. 

Media Use Consent (Optional)
I herby grant permission for films, video, and/or audio tape recording, slides and photographs to be taken of me during classroom instruction, skills assessment, and/or apprenticeship activities. I understand that this media will be produced and used for promotional, education, or programmatic purposes. I authorize the West Oakland Job Resource Center (WOJRC) to use my photograph on their website or in other official printed or electronic publications without further consideration. I further acknowledge WOJRC's right to crop or treat the media at their discretion. I also acknowledge that WOJRC may choose not to use my photo at this time but may do so at their own discretion at a later time. 

I also understand that once my image is posted on the WOJRC website or on other platforms or in other materials, the image can be downloaded by any computer user regardless of their affiliation to WOJRC. Therefore, I agree to indemnify and hold harmless from any claims WOJRC.
